Proof for qualified specialists with experience of at least 5 years: a diploma, certificate of residency or internship, a certificate, certificate of advanced training, employment history
Copy of the Working book of Chief of Medicine (experience not less than 5-years)
Marriage certificate if name changed
The lease agreement, floor plan legend, certificate of ownership
Contract with health institutions for advice, clinical, laboratory and hospital care
Contract with the organization for servicing medical equipment that is available to the licensee .
Notarized copies should be provided for all Founding documents and stuff professionals licenses.
